- 2.Fill in your name, address, email, and phone number
- 3.In the Privacy Choice dropdown, select Request to opt out of sale or sharing
- 4.Check the attestation checkbox at the bottom
- 5.Submit the form. DataAxle processes opt-outs within 30 days
