- 3.Select your state of residence from the dropdown
- 4.Enter your first name, middle name (optional), last name, street address, city, state, and ZIP code. Social Security Number is optional.
- 5.Check the boxes for the protections you want. At minimum select Opt out of the sale/sharing of my personal information. You can also check Opt out of profiling and Opt out of targeted advertising for broader protection.
